Where items instruct checking boxes, the individual options can vary depending on previous answers and so are not included here.

  2. Which research procedures does this study involve (please check ALL that apply)
  3. In non-technical language, describe the procedures subjects will be asked to complete or undergo. Explain step by step what subjects will be asked to do. If your study includes multiple variations of the procedures, please make clear the procedures included in the variations.

  2. Describe the criteria for enrollment – will you be limiting your enrollment to a certain age range, gender, people with certain health conditions, etc.? Please also describe any criteria that will exclude people from enrollment.
  3. Vulnerable Populations - check the boxes for ALL vulnerable populations from which you may enroll participants:

If the research site provides you a letter of permission, please attach here. (It is your responsibility to check with your research sites whether they must grant you official permission to recruit on their premises.)
